At the time of this incident last year, the root cause was unclear; however, once the official investigations concluded it became evident that inadequate training and poor supervision were key contributors to the release of live hydrocarbons.
Unfortunately, we have seen similar failures in the UK, where loss of primary containment has resulted from competency gaps and weak control measures. One case that stands out is the tragic death of a worker at the Lindsey Oil Refinery in 2010.
It is essential that operators implement robust mechanical joint integrity systems – processes that include clear flange tagging to identify joints scheduled for work, good supervision is maintained at all times and strict assurance that only qualified personnel with demonstrable competency assemble bolted flange joints.
Although the Martinez refinery incident did not result in fatalities, it serves as a stark reminder of the potential harm and disruption that can occur when industry standards and mechanical joint integrity guidelines are not rigorously applied across hazardous process environments.
Zulu Joint Integrity & Training Ltd exists to support asset owners in building and maintaining truly robust joint integrity programmes – programmes designed to significantly reduce the likelihood of incidents like these.
From compliance audits and independent flange inspection/QC, through to the delivery of industry‑approved joint integrity training and assessment, we are the UK’s largest independent subject matter expert in this field.
We work closely with our clients to ensure full regulatory compliance, strengthen operational assurance, and embed the competency standards that protect people, plant, and the environment.
Our mission is simple: to help prevent the unthinkable.
