The U.S. Chemical Safety and Hazard Investigation Board (CSB) has released a new safety video on its investigation into the fatal release of hydrogen chloride at a polysilicon facility in Charleston, Tennessee, that occurred on November 13, 2020.
The investigation highlighted several concerns and breaches of standards etc. but for us the issue regarding the importance of a joint integrity procedure that includes suitable tagging, independent inspection, qualifying competency of workers, recording of data and controlled means of bolt tightening really does ring true.
We encourage all our friends on here, particularly those working in high hazardous process industries to share the video to their teams.
This fatality could have been avoided – like so many others.
